Social Worker MSW Case Management

Ensures emotional and social needs of patients are met and maintained in accordance with facility guidelines and federal/ state regulations.  Leader of the discharge team and will work directly with the Case Managers and discharge planners to arrange safe, secure, appropriate and timely discharges.  Assist Case Management staff with patients that may demonstrate particular needs.

Qualifications

 

Regulatory Requirements:

·                     MSW

·                    BLS certificate.

 

Language Skills:

·                    Ability to read and communicate effective in English.

·                    Additional languages preferred

Skills:

·                    Basic computer knowledge.
